Alliance Jiu Jitsu Academia, INC Waiver

Waiver of Liability Alliance Jiu Jitsu Academia, INC urges all students to obtain a physical examination from your doctor prior to participating in any exercise program or using any exercise equipment. By signing this waiver, I nearby affirm that I  and/or my minor children are in good physical condition and do not suffer from any known disability or conditions that would prevent or limit my participation in this exercise program. I agree that by my, and/or my minor children’s, voluntary participation in this program, use of equipment and facilities of Alliance Jiu Jitsu Boise, INC shall be undertaken at my, and/or my minor children, sole risk. I hereby expressly forever release and discharge Alliance Jiu Jitsu Academia, INC its agents, affiliates, their respective administrators, directors, coaches, employees, other participants sponsoring agents, volunteers, property owner(s), and owner of premises of any claim, injuries, damages, actions or cause of actions and from all acts of negligence, active or passive and allotter fault the part of those listed above or property arising out of or connected premises where allianceJiu Jitsu Academia, INCis located whatsoever; from (a) the club’s negligence, (b) any other student’s. member’s, guest’s or invitee’s and activities, including but without limitation to students and/or my minor children’s use of the Alliance Jiu Jitsu Boise, INC parking lot, athletic facilities, athletic equipment, showers, or any facilities and activities associated with Alliance Jiu Jitsu Academia, INC. Jiu-Jitsu is a full contact sport and I fully understand that I and/or my minor children can incur serious bodily injury, either temporarily or permanently. Conditions may include but are not limited to heart attack, musculature and or skeletal injuries, dehydration, injuries to all or any joints, limbs, and face, or any other illness, soreness, or injury that I and/or my minor children may incur, including disfigurement or death. I and/or my minor children assume all risks injuries may suffer and responsibilities associated with using Alliance JiuJitsu Academia, INC athletic facilities, including any athletic activities, rooms, or other club usages. The undersigned assumes all the risks inherent and incidental to this type of sports activity as conditions for applying for admission to Alliance Jiu Jitsu Academia, INC. 

Applicable for all Law Enforcements Agents; I  hereby affirm that all techniques taught by Alliance Jiu Jitsu Boise, INC its agents, or volunteers, do not supersede department policy. 

Alliance Jiu Jitsu Academia, INC reserves the right to dismiss students at any time, for misconduct factions, which may convey an image of this school. The student agrees that he/she will refrain from using the knowledge obtained from Alliance JiuJitsu  Academia, INC with or without compensation, except for that student’s own personal growth and development. I agree and consent that Alliance  JiuJitsu Academia, INC and its nominees and assign, may use likeness and/or photography taken of myself and/or my minor child(red)  or a reproduction thereof, inane form, style, or color, together with many writing and other advertising or publicity material, including television and internet, in connection therewith, including the use of my and/or child (red)’s name in perpetuity, as they may select. The consent and release are given for valuable consideration and without limitation upon, or liability for, any use for advertising, illustration, publication, or broadcast of every kind,  or in trade or media or for any purpose. I further agree that such photography, and/or likeness, and the film, tape, plates, and negatives thereof, shale and remain the exclusive property of Alliance JiuJitsuAcademia, INC. I further waive any right to inspect or approve the commercial or advertising material. By sighing this waiver, I hereby certify and affirm that I have read and understand this agreement along with the terms and conditions of this document and will abide by the rules and regulations of Alliance Jiu Jitsu Academia, INC, and by sigh in this waiver, I am in freeman with all terms and conditions. If the student is under 18 years of age, consent in the form of signature below must be given by a parent or guardian. by signing this waiver I hereby affirm I am the legal guardian of the minor person referred to above, and Agree and consent to the foregoing insect capacity, as well as individually.
